TCM tips for De Quervain’s tenosynovitis (mommy wrist)

Thumb-side wrist pain when you lift or grip often means the thumb tendons are overworked. A splint, clinic care, and gentle stretches at home usually help together.

In brief

Pain on the thumb side of the wrist when lifting, gripping, or twisting often reads as De Quervain's tenosynovitis, also called mommy wrist (妈妈手, Mother's Hand). L-shape baby lifts, mouse work, phone, gaming, and garden tasks are common loads. Finkelstein screen: tuck the thumb, make a fist, bend toward the little finger; pain at the thumb base is a positive screen, not a diagnosis. Clinic care uses acupuncture, tuina, and herbal plasters. At home: a splint for 4–6 weeks, a palm-up lift under the baby's bottom, and opposition plus grip exercises.

01 · What it is

What mommy wrist is

妈妈手 · Mother’s Hand · mommy wrist

Pain on the thumb side of the wrist when lifting or gripping often sits in the two tendons that lift the thumb. They glide through a small tunnel at the wrist; when the sheath inflames, the thumb catches and loses range.

It is 妈妈手, also called Mother’s Hand or mommy wrist, because it is common in new mothers. Hormonal change plus repetitive lifting with the hands in an L shape (fingers on the baby’s back, thumbs on the chest) loads those two tendons.

What loads the sheath

Chronic overuse of the wrist is the usual driver. Any activity that repeats thumb or wrist motion can irritate the sheath.

Everyday loads on the thumb sheath
Trigger What loads the tendons Typical picture
Lifting a babyHands in an L shape, many times a day. Thumbs take the chest; fingers take the back. New mothers, especially in the first months.
Mouse, phone, or gamingProlonged grip and thumb motion. The sheath stays under a small, repeated pull. Pain after a long desk day or a long scroll.
Garden workRepeated wrist and hand tasks. Grip plus ulnar and radial deviation. Soreness after weeding, pruning, or potting.

What the thumb-side wrist often feels like

Pain sits on the thumb side of the wrist, not in the palm. It may travel into the forearm.

1. Pain at the thumb base

Swelling, soreness, or pain on the thumb side of the wrist. Pain may travel into the forearm.

2. Worse with grasp

Picking up a mug, a phone, or a child loads the sheath. Onset can be gradual or sudden.

3. Snap or stick

A snapping or sticking feel when the thumb moves, and difficulty moving the affected wrist or thumb.

02 · Screen

The Finkelstein screen

Thumb in palm · Fist · Ulnar deviate

This three-step check is the Finkelstein screen. Pain or tenderness at the base of the thumb is a positive screen. It is not a diagnosis; a physician confirms.

1. Tuck the thumb

Bend your thumb so it rests across your palm.

2. Make a fist

Close your fingers over the thumb.

3. Bend toward the little finger

Bend the wrist toward the little finger (ulnar deviation). Pain at the thumb-side base is a positive screen.

Signs that need urgent care

These signs sit outside a typical mommy-wrist picture. Get assessed the same day.

1. Hot swollen wrist

A hot, red, swollen wrist, especially with fever.

2. Spreading numbness

Numbness or weakness that spreads into the hand or forearm.

3. Cannot move the thumb after a fall

Sudden inability to move the thumb after a twist, fall, or impact.

03 · Clinic methods

How TCM treats

Overworked thumb tendons often inflame the sheath, especially after L-shape lifts or repeated grip. Postpartum cases may also sit with Blood and qi load. A physician matches methods to cause, signs, and duration after pulse and tongue.

Clinic methods for mommy wrist
Method What it does for the wrist When it sits in the plan
AcupunctureNeedling to move blood in the affected area. Eases pain and inflammation in the sheath. Often first-line when the thumb-side wrist is angry.
Tuina massageLocal acupressure on the affected area. Relieves pain, reduces inflammation, and improves blood flow. When the tissue around the sheath stays tight.
Herbal plasters and ointmentsExternal application only. Further eases local pain and supports healing of the sheath. After a physician names the pattern.
Lifestyle modificationsHow the thumb is loaded each day. Splint, rest, a different baby lift, and gentle home stretches. Alongside clinic methods, not instead of them.

When to book for mommy wrist

If thumb-side pain has lasted weeks, or a splint and rest are not settling it, a physician can name the pattern and decide whether acupuncture, tuina, or plasters belong in your plan. Postpartum cases can also start from the Women's Health programme.

04 · Daily load

Unload the thumb

A splint immobilises the thumb and wrist for 4 to 6 weeks so the sheath can quiet. Rest from repetitive thumb and wrist motion sits alongside it.

How to unload the thumb sheath
Habit What it does What to try
Wear a splintFour to six weeks. Supports and immobilises the thumb and wrist. A thumb-spica style brace that leaves the fingers free.
Rest the handPause the motion that started the pain. Repetitive thumb and wrist tasks keep the sheath angry. Shorten mouse, phone, and garden sessions while it settles.
Lift the baby differentlyPalm up, under the bottom. The L-shape lift loads the two thumb tendons. Keep the palm up and lift under the bottom, not under the arms.
Check breastfeeding positionA pillow takes the head weight. The baby’s head resting on the hand loads the thumb. Use a pillow so the full weight is not on your hand.

05 · Home care

Two home moves

Opposition stretch and a soft-ball grip are the two home moves. Hold each 5 seconds; repeat 10 times a hand. Ease off if the stretch turns sharp.

FAQs

Mommy wrist questions

Common questions on 妈妈手, the Finkelstein screen, splints, home exercises, and how this differs from carpal tunnel.

What is mommy wrist in Chinese?

Mommy wrist is 妈妈手, also called Mother’s Hand. In Western terms it is De Quervain’s tenosynovitis: inflammation of the two tendons that lift the thumb (abductor pollicis longus and extensor pollicis brevis) where they pass through a tight sheath at the thumb-side wrist.

Can acupuncture help De Quervain’s tenosynovitis?

Acupuncture is commonly used for De Quervain’s. Fine needles are used to ease pain and support blood flow in the inflamed sheath. A physician matches needling to how the wrist reads, often alongside tuina and herbal plasters, plus a splint and a change in how the thumb is loaded.

How do you test for mommy wrist?

The Finkelstein screen is the three-step check on this page. Bend the thumb across the palm, close the fingers over it, then bend the wrist toward the little finger. Pain or tenderness at the base of the thumb is a positive screen. It is not a diagnosis; a physician confirms.

How long should I wear a splint for mommy wrist?

A thumb-and-wrist splint is commonly worn for 4 to 6 weeks. It supports and immobilises the joint so the sheath can quiet. Rest from repetitive thumb and wrist motion sits alongside the splint.

Why is it called mommy wrist?

It is common in new mothers. Hormonal change plus repetitive lifting with the hands in an L shape (fingers on the baby’s back, thumbs on the chest) loads the two thumb tendons. Palm-up lifts under the bottom and a breastfeeding pillow reduce that load.

What exercises help mommy wrist?

Opposition stretch and a soft-ball grip are the home moves taught here. Touch each fingertip, hold the little-finger touch 5 seconds, then squeeze a soft ball 5 seconds, 10 times a hand. Clinic tuina is different: a physician works the local tissue after naming the pattern.

Is mommy wrist the same as carpal tunnel?

No. Mommy wrist (De Quervain’s) sits on the thumb side of the wrist, in the two tendons that lift the thumb. Carpal tunnel sits on the palm side, where the median nerve is compressed. Thumb-side pain with a positive Finkelstein screen points to the sheath; palm numbness or night tingling in the thumb, index, and middle fingers points to the nerve. A physician distinguishes them.
